Medication
Assistance Program
 The
Medication Assistance Program (MAP) provides medications for
low income people with chronic conditions at no charge or
for a nominal fee. This is accomplished through
applications to the pharmaceutical companies which are
filled out by our MAP volunteers. MAP volunteers ensure
that the proper signatures and documentation are included
and submit them to the pharmaceutical companies.
Medications for Clinic patients are received and distributed
by the MAP volunteers.
To participate in the program,
patients must qualify for CCA services and are referred to
MAP through the AHC (Adult Health Center) or by a CCA
Caseworker. Patients of other Doctors who meet the
qualifications and are referred by a caseworker may also be
in the program. People who have Medicare, Medicaid, or any
private prescription insurance do not qualify for MAP.
