Services:
Qualifications:

Must be 19
years or older
-
Working poor; uninsured or underinsured
with limited or no insurance; no Medicaid or Medicare unless
patient is requesting dental or eye services
(Medicaid/Medicare does not provide eye/dental care).
- Live in Denton County and
in one of these school districts; Lewisville, Argyle, Lake
Dallas, Northwest, Coppell, Frisco, Little Elm and Denton
County portion of Carrollton/Farmers Branch ISD
Prescreening Interview Required:
Will need to bring the following
documents:

Proof of gross annual income
-
Proof of Denton County residency
(lease ,mortgage coupon or landlord letter, or current
utility bill in your name)

Social
security card and driver’s license or picture ID.

 Services Offered
Medical and Dental Primary care
Includes the diagnoses,
treatment, medications, medical supplies/equipment,
laboratory testing, radiology procedures and follow-up
care.
The conditions most commonly
treated are hypertension, diabetes, respiratory
infections and heart disease.
